As a Clinical Nurse Specialist for Older People you will need to demonstrate a good understanding of how to manage the complex care needs of older people and be willing to develop the service in line with the Enhancing Care in Care Homes framework, and work collaboratively to support the PCN s to enable the care homes to feel part of the community health family.

Having excellent assessment and communication skills and being able to articulate key information to patients and families is a key requisite for the role, as the CNS role supports the Pathway 3 assessment process from the Hospital Discharge and Community Support; policy and operating model. As a Band 7 Clinical Nurse Specialist you will use your clinical experience, expertise and advanced skills to provide leadership and effective caseload management, working closely with users of our services and other stakeholders. Band 7 Clinical Nurse Specialists are expected to be kind and responsive but professional and informative and contribute to the quality of the services provided across the health economy by:

Keeping the people who use our services as safe as possible through the use of sound clinical skills and effective risk assessments
Ensuring the best possible clinical outcomes by using up-to-date skills and using evidence-based policies and procedures
Ensuring the people using our services have a good experience by respecting, empowering and working in partnership with everyone involved in care planning.
