Job Summary
We are looking for a motivated and experienced registered nurse with a warm personality, alongside excellent communication and interpersonal skills. As an integral member of the team, you will work together to provide nursing assessments for patients who have returned home from hospital or who have been unwell at home and the goal is to keep them at home with rehabilitation support. You will support colleagues in the delivery of compassionate, safe and structured care, you will be enthusiastic to share your knowledge when managing and mentoring more junior team members.
The role of the Intensive Community Rehabilitation (ICR) Nurse is to conduct the nursing element of the assessments and nursing procedures for the patients on the caseload as well as exercising leadership and management of a defined nursing team within the East Integrated care teams. To deliver skilled evidence-based nursing assessment and relay this information to the other members of the MDT in a timely and concise manner. To ensure safe clinical practices and high standards of care are maintained to meet service demand.
This role may require supporting in the running of catheter clinics, so knowledge in this area is desirable.
Service provision is during working hours between Monday to Friday. This role requires a full driving licence and access to a car as most of the workload will be visiting patients in their own homes.
Main duties of the job
- Conduct the nursing element for our patients, to include patients who may for example be frail, at risk of falling, have pressure damage or unwell etc
- Triage patients to ensure referrals meet the service criteria and identify when the patient needs to be discharged from the service appropriately.
- Meet the service demands including reprioritising clinical and non-clinical workload, applying contingency plans and flexing the service, and where necessary escalating to the Team Lead.
- Provide clinical support, leadership and guidance to junior staff, including Band 5 nurse, health care assistants and Multi- Therapy Assistants.
- Communicate effectively with patients, carers, family members, including those with communication impairments, challenging behaviour or cognitive deficits, as well as those whose first language is not English.
- Documentation to be completed for all interactions regarding patients in a timely manner on RiO, ensuring detailed, accurate and confidential patient records according to Trust policies
- To ensure the workload is evenly distributed amongst the nursing team and delegate work as deemed necessary.
