We currently have a band 5 secondment vacancy for 12 months with an exciting opportunity to extend the contract to cover maternity leave the following year in the Hamworthy and Upton D/N team. The hours available are 30 hours a week, would consider less. Excellent communication skills are required when caring for patients in their own home. Any clinical skills required for this post will be supported.

Work as a registered nurse as and when required with community nursing teams, providing evidence based, high quality, holistic nursing care for patients in their own homes.
Carrying out nursing assessments, treatment and care and liaising with other agencies as appropriate.
To be developed and supported by Dorset healthcare with training and development to support a registered nurse with caring for patients in the community setting.
To assess, plan, deliver and evaluate nursing care for patients to meet their specific needs.
To assess the needs of patients in a variety of settings and formulate a plan of care. This may involve liaison with carers, other health professionals, GP’s, consultants, hospital nurses, social workers, hospice staff, specialist nurses, voluntary and statutory workers.
To provide evidence based nursing care to a defined patient caseload with the support of the District Nurse.
