The Adult Learning Disability Team Supports Adults with Learning Disabilities to live a fulfilled life with access to social care services as required. This is a rewarding area of social work, with a focus on person centred planning and outcome focused practice. We work with a high number of people with varied needs ranging from complex health to forensic needs, who require skilled interventions. The team is a specialist service working alongside and in partnership with Cheshire and Wirral Partnership NHS trust. The role is diverse, and no two days will be the same. The candidates will require the ability to form relationships with service users, families, and other professionals to allow for successful outcomes. This is an experienced team of highly motivated, hardworking, and skilled professionals, many of whom are longstanding in the service.

You will have:
- NVQ Level 3 in Health and Social care or equivalent (Desirable).
- Be required to undertake an enhanced DBS plus Adults Barred List (Essential).
- Experience of working with vulnerable people and their carers.
- Ability to communicate effectively with customers, carers and families.
- Awareness of importance of multi-disciplinary working and an integrated approach.
- Problem solver with good planning and organisational skills.
- Ability to relate to other disciplines in a sensitive and professional manner.
- Some knowledge of assessment and support planning within legislation.
- Ability to cope positively with change and an ability to promote positive changes to make a difference.
- You are confident, energetic and enthusiastic as well as sensitive and resilient.
- Self-motivating individual with the passion to make a difference in adult social care.
- Resilient personal stress management.
This is a hybrid role where you will be based at our offices across the borough as well as the opportunity to work at other locations, complete visits and work from home.
