Acute Hospital at Home Service - DCHFT

An opportunity has arisen to work in an exciting integrated model for the delivery of patient care in Dorset County Hospital’s Acute Hospital at Home service. This service proactively manages and monitors a defined caseload of patients who have complex care needs in the short term, on a multi-disciplinary basis in their own home.

Patients are offered continuity of care by following them from hospital into the community and the post provides the opportunity to work more closely with community and primary care colleagues in a seamless service.
This is a service with the patient’s needs at its heart and aims to support admission avoidance and early discharge. The focus of the service is to support the acutely medically ill in their own homes by using inpatient level treatments, diagnostics and skills.
We are looking for dynamic experienced generic Registered Nurses to work in our multi-disciplinary team, ensuring a patient focused and individualised approach. Ideally you will have acute hospital experience as well as experience of effective multi-disciplinary team working in its widest sense. Car Driver Essential.
Typical Daily Tasks Will Involve
- visiting patients at home
- giving intravenous medication
- holistic assessments
- complex wound management
- patient and family education (i.e with peg feed)
